Services include:

• Care plan assessments to identify problems, eligibility for assistance, and need for services.

• Screen arrange, and monitor in-home help or other services.
• Review financial, legal, medical issues, and offer referrals to geriatric specialists to avoid future problems and conserve assets.
• Act as a liaison to families at a distance, alerting families to potential problems.
• Evaluate client's home for necessary modifications to maintain client safety and supervise those modifications approved by the client
• Assist with placement options, including moving a client to or from a retirement complex, assisted living facility, or nursing home.
• Provide consumer education and advocacy.
• Assess for eligibility of entitlements, including Medicaid, VA, and private insurance.
• Process applications and claims for entitlements.
• Oversight of medication compliance.
• Bill payment and bank statement reconciliation.
• Maintain monthly and annual financial records.
• Facility/Home visits to assess client and ensure quality care.

Nursing services include:

• Individualized health care plan, implementation and monitoring.
• Medication management, including set-up and monitoring compliance.
• Complete head-to-toe nursing assessments
• Liaison between physician, client, and family including attendance at physician appointments.
• Family caregiver education.

All services include a written report sent to the client and/or designated family member.
